This achievement is so awesome it definitely needs a representation as an in-game thing, be it a monument or novelty item, which I would definitely have to get if it’d be available!
The activity tracker is an absolutely must too, of course.

This is one of the few accomplishments you can do solo that merits a monument of some sort. It speaks to our need to wonder and explore, which is a celebrated part of the EVE experience.

Did you use scouts, escorts, or different fits?

I only have my word on my honor, I didn’t use an Alt scout or had anyone scouting ahead for me. It wasn’t until the last 600 wormhole systems that I sought out help and my corp, Signal Cartel, unknowingly assisted me in an corp event entitled “The Great Hunt”. Only a handful knew what was actually going on.
For known space, I just flew clever. I mapped out my route, gathered my intelligence via the in game map of kills, jumps, etc, other sources like Dotlan, zKillboard, Eve Who. Basically, know your space, where you’re at, where you’re going. I took my time, I was in no hurry.
Wormhole space… I was just damn lucky!

But the ingame Completion system keeps track of that data.
In fact, that is one of the reason it was designed for.
It keeps track of how many systems you visited under Exploration, systems explored, constellations explored, and regions explored.

I started my journey nine years ago long before the in game activity tracker was a thing end of last year. 
So, spreadsheets was my chosen method of tracking it manually.

Ah yes, they don’t have the previous data, only the one after their system was implemented.
