There are regions of space where CONCORD doesn’t exist and where people don’t care about outlaw status or suspect status at all.
I live in such regions, I should know. Only when I go to CONCORD patrolled space I need to care about those statuses.
There are also systems without players, but that doesn’t mean those players don’t care about suspect status or outlaw status, it only means there are no players to care.
But why are we talking about space where CONCORD does not exist , when the topic is about high sec ?

It is not an NPC thing , because the actions performed to get the flag are done by you or other players.

Capsuleer log off timer? You cannot log off safely as fast as you could without, to svoid players logging out in the face of danger.
Weapons timer flag? You cannot gate or dock up to avoid the fight.
Jump timer flag? Your next jump will take some more time so you cannot easily chain jumps. Jump fatigue? Another flag, but now to make fast chaining jumps less efficient.
All flags have a meaning, otherwise there is no point in keeping them around.
For suspect and outlaw flags, those flags mean how NPCs will (or will not) react.

Indirectly some players may not attack you when they know there will be an NPC response, but many players don’t care about that and will attack you anyway.

A flag is a flag. It just means “this entity verifies this flag”.
What you are talking about, is rules linked to specific flags. I agree that a flag without an effect linked to it seems useless.
For rats the suspect flag is useless. It bears no meaning to them. Does not mean that the flag is meaningless intrinsically.

If you are NOT suspect, and I shoot you, even without concord, I get a criminal flag and can’t warp, can’t take gate, can’t cloak, can’t dock for 15min.
If you are suspect, and I shoot you, I don’t get this flag.

So first you are wrong in your definition of meaning, because flags intrinsically bear no meaning - the meaning is through the mechanism they are associated with ; then you also are wrong in the fact that it only interacts with NPCs.
Mechanics-wise with regards to access it, it is not - any timer that denies docking denies a tether. If you are eligible to dock at the station, you are eligible to receive tether.
