It’s just servers, no miracle machines. Sometimes odd things happen.

Probably a socket closure.
Stuff happens, as Boldly Gone points out computers are simple pieces of equipment and failures are not unheard of. Considering all that has to happen to be able to connect to a game it should not surprise you that this happens from time to time.
If you lost a ship during a PvE activity as a result of this file a support ticket with all of the relevant information you can give them, date, time, your location etc and see what happens they have replaced ships in similar circumstances in the past.
