Cockroach is a joke ship by design. Polaris is just… meh, it looks too simple, but not ugly.
Imicus, Helios, Lachesis, Celestis, a lot of gallente ships really lol
The Helios being so ugly is part of why I skilled into Caldari T2 frigates, so I can use the Buzzard which doesnt look like a turd

Personally I like the Sansha ship line… except for the turd… I mean carrier.

The Imicus, and the Minmatar Shuttle are some of the ugliest ships in the game.
The Minmatar Slasher is also ugly! 
the Minmatar shuttle looks like a war ship.
