I don’t think he left on good terms. Wasn’t blackout his idea? When it turned out to be a disaster, CCP decided to fire him, despite Falcon, being PvP-focused bittervet still thinking that blackout was great (it wasn’t, because there were far more fewer targets for hunters).

I could be wrong but I don’t think Blackout was his idea, everything that’s implemented into the game has to be approved by the CEO, Hilmar Veigar Pétursson. If anything, the success or failure of any action pertaining to the game solely rests on him.
As for CCP Falcon, it definitely looks like he left on bad terms but I don’t think he got fired, I think he just got fed up with the BS and quit after CCP announced the end of Blackout.
