I suspect (no pun intended) that particular scenario might just never been thought of at the time by the creator of the diagram. As it is, I think that is the second version of it. I never noticed if there was another version after that.
It is a relatively recent change, due to some remote repair shenanigans from neutral alts during wars. Keeping your safety set to yellow will prevent it, at least!
